The Beaumont Public Health Department is
continuing its transition into the new millennium. The department has
begun to assess the needs of the community as it relates to public
health. Community-based activities are becoming the major focus of the
department. Recognizing the new role that must be established to ensure
a healthy community, the department created partnerships with various
public and private entities. The department serves as a training site
for students at Lamar University acquiring Bachelor degrees in nursing,
nutrition and health promotion, also providing preceptorships for
graduate nursing, biology and nutrition students.

ADMINISTRATIVE SERVICES
Administrative Services provides
guidance and direction to the various divisions of the department
through the monitoring of established goals and objectives. The section
implements corrective action when deemed necessary. Additionally, data
collection and analysis are integral parts of its role.
PUBLIC HEALTH PROGRAMS
Public
health programs with the Health Department provide a variety of
services, including communicable disease controls, which encompasses the
investigation and surveillance of infectious diseases including
tuberculosis, sexually-transmitted and vaccine-preventable diseases and
other reportable diseases. Immunization of residents is also a part of
communicable disease control and is done in a variety of settings:
health fairs, public schools, day care centers and community sites such
as one of the Police Department's substations.
I n
addition, preventive child care, which includes physical assessment, lab
screens for anemia, sickle cell, cholesterol and lead, as well as
assessment of growth, development and mental health status are provided.
As a component of child health, the department provides investigation
and surveillance of elevated lead screens and offers a comprehensive
lead prevention education program. A lead education specialist is
available for community group presentations.
Sexually-transmitted disease (STD) services are provided in a
confidential manner by competent staff who are prepared to assess,
diagnose and treat a variety of STDs. Follow-up investigation and
surveillance of STDs is carried out by a staff of disease intervention
specialists (DIS). Chronic disease screening for hypertension and
diabetes are available to the general public and are available on a
walk-in basis. Referral to a physician is made when abnormal values are
detected.
The
City's employee health program has the responsibility of administering
pre-hire drug screens, preliminary screening for annual and specialized
physicals, adult immunizations and TB screening, as well as,
administering the occupational exposure program for safety sensitive
positions.
The
Beaumont Public Health Department works in conjunction with the Texas
Department of Health in issuing, at a cost, rabies vaccine for pre and
post-exposure to potentially rabid dogs.

VITAL STATISTICS
The
Vital Statistics Division files and maintains exact photo copies of
the original birth and death records that occur in the City of
Beaumont and Precincts I and VI. We can be reached at (409)
832-4000. Hours of operation are Monday - Friday, 8:00am - 5:00pm.
Vital
Statistics issues certified copies of birth certificates, death
certificates and computer generated birth certificates of
individuals born in the State of Texas only. Years available are
1926 to present. This office also provides general information to
the public concerning vital statistics of all states. We also assist
individuals in filing delayed birth certificates for births in the
State of Texas as well as delayed death certificates in local
jurisdiction only.
Forms
may be obtained from Vital Statistics for amendments and
paternities. Forms must be notarized before mailing to the State
Bureau of Vital Statistics in Austin, Texas. Notary service is
offered in Vital Statistics when a notary is available.
Confidentiality
Birth
Records
Birth records are confidential records for the first 50 years
after filing and death records are confidential for the first 25
years. These records may only be issued to a properly qualified
applicant upon submission of a written application that fully
identifies the records.
Adoption
Records
Adoption records are confidential forever and may only be opened
upon order of the court which granted the adoption. These
records are opened at the State Bureau of Vital Statistics.
Paternity
Records
Paternity records are confidential and may only be obtained
through the State Bureau of Vital Statistics by court order from
the Judge of a paternity suit.
Parents' names, social security numbers, health and medical
information are always confidential, regardless of the age of the
record.
Qualified Applicants Only qualified applicants may request birth or death certificates in
person or by mail request. A qualified applicant is an immediate
member of the family or a legal representative and the registrant. A
written application must be submitted which fully identifies the
record. Applicant must present positive identification, such as a
driver's license, picture id, etc., or two documents, one that bears
the name and signature of the applicant.
Payment For Services Payments that are acceptable for services include cash, personal
checks, cashier’s checks, or money orders with proper
identification. Payment is to be made to the Beaumont Public Health
Department. Personal checks must contain pre-printed name and
address. Two-party checks, checks greater than the amount of
purchase, and out-of-state checks will not be accepted. Sorry, no
credit cards.

The Health Promotion and Education
Division provides a valuable service to the community
through outreach programs geared towards the promotion of
good health through health education with a broad range of
health topics from wellness to specific health problems,
such as diabetes and hypertension.

with the Lamar University Health Kinesiology/Family &
Consumer Science AP4 Program and Nursing.
